No School Fun Days & Camp Registration

Please complete this form in it’s entirety and be as detailed as possible as this is the information we share with our program staff and therapists. Some of our programs partially rely on grants for funding, and certain pieces of information are collected for the purpose of required grant reporting. Client privacy protection will be observed in accordance with those rules set forth in the Health Insurance Portability and Accountability Act (HIPPA).

Summer Camp 2026

9am - 3pm | Ages 5-17 & 18-30

Friendship Campus | 8700 Sw 112th St, Miami, FL

Lunches MUST be NUT FREE. 

Sign up for ALL 4 weeks in June, and you automatically receive $200 off total price.

Registration will close the Thursdays prior to the week. NO EXCEPTIONS.

Are you a currently registered for the 2025-2026 year?
Please list fidget/sensory toys the participant enjoys so we can have those available here for them.
Please describe in full detail what they are allergic to.
Do they carry emergency medicine on them?
If you child carries an EPI- PEN you MUST carry Benadryl to administer as well. Friendship Circle Miami WILL ONLY administer NASAL SEIZURE SPRAY AND EPI-PEN, we WILL NOT administer daily medications, NSAIDS or rectal seizure spray. In the event of a seizure or epi-pen is required 911 will be called as well as Primary Contact.
Is your child potty trained?
Can your child be in photos?
Will your child be accompanied by their Aide/RBT? If so, we MUST have a copy of their background check and signed contract PRIOR to camp starting.
Summer Camp: June 8 - July 3: ALL 4 WEEKS! (Monday - Friday)
Use this option if you are going to sign up for ALL 4 weeks of Summer Camp, Monday - Friday. You receive $200 off total price, price is already reflected with discount.
Summer Camp Week 1: (Monday - Friday)
Registration will close Thursday, June 4.
Summer Camp Week 2: (Monday - Friday)
Registration will close Thursday, June 11.
Summer Camp Week 3: (Monday - Friday)
Registration will close Thursday, June 18.
Summer Camp Week 4 (Monday- Thursday)
Registration will close Thursday, June 25. FRIENDSHIP CAMPUS WILL BE CLOSED ON FRIDAY JULY 3RD.
Summer Camp: Week 5: August 3rd - 7th
Registration will close Thursday, July 30.
Did you apply for scholarship?

We are looking forward to a wonderful and successful camp experience for your children. To help ensure everything runs smoothly and safely, we kindly ask that you review and respect the following camp policies:

1. No Refund Policy
We are unable to provide refunds if a child misses camp due to illness or does not participate in a scheduled activity, such as a field trip. All buses, venues, and related expenses are prepaid in advance. We appreciate your understanding and ask that you please refrain from requesting exceptions.

2. Medication and Creams
Camp staff are not permitted to administer medication or apply creams. Please take care of any necessary treatments before or after camp hours. In the case of minor cuts or scrapes, we will clean the area with soap and water and provide a bandage.

3. Medical Emergencies
In the event of a serious medical emergency, such as a seizure or anaphylactic reaction, we will immediately call 911 and then notify parents.

4. Illness Policy
If your child is not feeling well, please keep them home and inform us. This helps protect the health and well-being of all campers and staff. Specifically, please do not send your child to camp if they have a fever, persistent coughing, or green mucus, as these may be signs of a contagious illness. If a child becomes ill during the camp day, we will contact you to arrange for prompt pick-up. We appreciate your cooperation in helping us maintain a healthy environment for everyone.

5. RBT Support
If your child attends camp with a Registered Behavior Technician (RBT), the RBT must remain with the child for the entire duration of the camp day.

6. Phone Policy & Communication
Please do not send your children to camp with phones. If a phone is brought, it must be kept away at all times during camp hours. If you have any questions or concerns throughout the day, please don’t hesitate to reach out to the Program and Family Manager directly.
